COMMUNITY HEALTH ADVOCACY & RESOURCE TEAM (CHART)

We are a group that collaborates with the community to impact policy, systems, and environmental change to raise the overall health of Clatsop County residents. The environment is not just about air and water quality. Sometimes thinking outside the box is helpful in understanding environment in a health setting. A healthy environment is so broad because it encompasses everything you do from work, to play and everything in between. Community health is important and should be to you also. Just by being alive you are impacting your health as well as the health of others that surround you.

CHART uses the Social Determinants of Health as a guiding map of where and how to focus its efforts.

Social Determinants of Health

Social determinants of health (SDOH) are the conditions in the environments where people are born, live, learn, work, play, worship, and age that affect a wide range of health, functioning, and quality-of-life outcomes and risks. .

SDOH can be grouped into 5 domains: Economic Stability; Education; Health and Health Care; Social and Community Context; and Neighborhood and Build Environment.

CHART is a multi-sector community collaborative focused on improving the health of Clatsop County residents. CHART operates in the background and helps create alignment among agencies and projects.

 

CHART meets monthly on the second Wednesday of each month, 9:00-11:00 AM

Location: CHART is meeting Via ZOOM for the duration of the COVID-19 pandemic.

Our pre-pandemic meeting place was held at the beautiful Fort Clatsop Lewis and Clark National Historical Park - Netul Conference Room, located at 92343 Fort Clatsop Rd, Astoria, OR 97103
